Respite Center

Our Valley Oak Respite Center provides an interactive program for frail Seniors and those afflicted with Alzheimer’s, other related dementias and physical disabilities. Seniors have the opportunity to participate in meaning activities which encourage independence and fellowship. The program is held Monday, Wednesday, and Friday from 10:30-2:30pm in Danville, CA.

Durable Program

Diablo Valley Foundation for the Aging offers seniors age 60 years or older a yearly service called our Durable Program.   Seniors enrolled in this program appoint Diablo Valley Foundation to act as their legal representative. Our staff makes decisions on their behalf if they are incapacitated or unable to do so. We ensure their Advance Directive wishes are protected and treatments are administered just as they want.

 

Our Services Include:

 

1. 24-hour Emergency Call System. Diablo Valley Foundation provides 24-hours a day, 7 days a week, on-call emergency services to advocate on your behalf in the case of an urgent situation. Our care management team will ensure your health needs are met during an emergency situation.

 

2. Two Personal Visits. You will receive two visits annually. The first visit is designed to discuss your health care needs and concerns, as well as determine any specific wishes you want carried out by DVFA.

 

Approximately 6 months later, we will meet with you again to review your health and overall well-being. Our Care Managers will discuss any new issues or changes you may be experiencing. This may include financial matters, health issues, emotional problems, transportation troubles or other concerns.

 

3. Phone Consultation Services. Our staff is available for periodic phone calls to answer any questions you or your family may have in between our semiannual personal visits.
